Air Force Safety Center (HQ AFSEC) records
HQ AFSEC is a separate RSC, thus any request for Air Force mishap records related to Aviation, Missiles, Weapons, Space, and Ground accidents must be requested through HQ AFSEC, Kirtland AFB, NM. More specifically, HQ AFSEC maintains all Safety Investigation Board (SIB) Reports for Air Force mishaps from 1956 to Present. SIB Reports created prior to 1956 must be requested from the Air Force Historical Research Agency (AFHRA), Maxwell AFB, AL. If the requester is asking for a publicly releasable report, they can request for a copy of the Accident Investigation Board (AIB) Report (maintained by MAJCOMs for 25 years) from the respective MAJCOM, who accounted for the mishap. Again, the AIB is a publicly releasable report, whereas the SIB is a privileged safety report that would need to be redacted extensively prior to release.
